Protection of patient/patient confidentiality is of the utmost importance to Harris County Public Health & Environmental Services (HCPHES).  Protecting the confidentiality assures that our patients, families and communities have the trust necessary to collaborate with us regarding preventive services, treatment, and disease investigation.

  • All HCPHES staff working with your health information receive special HIPAA training that teaches them how to handle and protect your confidential information.
  • Visit our Notice of Privacy Practices (available in English and in Spanish) for descriptions of how your health information may be used and disclosed.
  • Your rights under HIPAA include:
    • Right to know what will happen to your protected health information
    • Right to examine or request a copy of your protected health  information
    • Right to request corrections to your protected health information
    • Right to sign an authorization form releasing/requesting protected health information
    • Right to request limited access to your information as allowed by HIPAA
    • Right to cancel a signed authorization
    • Right to file a complaint at web site http://www.hhs.gov
