Immunization Outreach Mobile Team (IOMT)

IOMT Schedule                                           IOMT Request Form

The Immunization Outreach Mobile Program was created by Harris County Public Health & Environmental Services (HCPHES) in 2003.  The program was designed to increase the immunization coverage rates among children aged 0-35 months residing within HCPHES jurisdiction, excluding the City of Houston.

As of January, 2008, the IOMT provides most vaccines to qualified children (children on CHIP, Medicaid, no insurance or under-insured) and adults (no insurance or under-insured) at no cost.

An adult must be present so consent can be obtained for all minors (age 17 and younger).  Individuals needing shots must bring all of their immunization records to assist the staff in determining which vaccines are required during the IOMT visit.

The IOMT van is available upon request to provide immunization services during various community outreach events.  Requests must be submitted 8-10 weeks prior to the event.  Requests received more than 10 weeks before the event will have to be resubmitted.

Please FAX all completed IOMT request forms to 713-439-6324.
